DD Form 214 Certificate of Release or Discharge From Active Duty. If you were discharged from active duty ater January 1, 1950, a copy of this certificate should be included with your Certificate of Eligibility when applying for loan eligibility. If you do not have your DD Form 214, you should contact the National Personnel Records Center.
